We thank you for contacting our office for your dental healthcare needs. Our commitment to you is to provide personalized dental treatment that will result in the ultimate smile, optimal oral care and comfort for you as an individual. 

If you have any questions for us prior to your appointment, please do not hesitate to call. Please give us a 48- hour notice If you need to cancel or reschedule your appointment.  Unfortunately, due to the increasing numbers in missed appointments, we are forced to implement a “NO SHOW/MISSED APPOINTMENT” policy.

Effective 7/15/21 there will be a $50 charge for no show/missed appointments .

To ensure that each patient is given the proper amount of time allotted for their visit and to provide the highest quality care, it is very important for each scheduled patient to attend their visit on time. F or any questions or further details, please speak any of our staff.

Please print, complete and bring the attached forms and your current dental insurance cards with you to your first appointment. When you arrive, please let the receptionist know that this is your first visit to our office.
